Insurance Information

patient and staffPatients with health insurance will need to bring an insurance card and any employer-required insurance forms to the appointment. Patients will be asked to assign benefits from the insurance company directly to the hospital or physician group providing treatment.

Deborah submits bills for services provided to the insurance company and does everything possible to expedite the claims. Services are billed under the name, Professional Service Fund, and include all physician specialties. An insurance policy is a contract between the insured and the insurance company, so patients may be asked to assist in obtaining payment.

Insurance Requirements

Depending on the insurance provider and selected plan, special requirements – such as a second opinion or pre-certification for tests or procedures – may be necessary in order for services to be covered. It is the responsibility of the patient to make sure the requirements of the plan have been met. Otherwise, admission may be delayed. Patients are responsible for initiating and obtaining a referral to a specialist if required by the HMO. A referral must be obtained from the patient’s primary care physician if covered by an HMO, PPO, or other managed care plan. Call 609-893-1200, ext. 5732 with any insurance-related questions.

Patients Without Insurance

Patients without insurance can discuss arrangements with a representative of the Patient Access Services team. Patients may be asked to submit personal financial documents, which allow Deborah to be reimbursed, in part or in full, by the state for any uncompensated care provided.
